I agree to make payments to Melissa Parenteau, MA, LPC for the counseling services rendered in the amount of $85 for each 45-50 minute session. This amount will be pro-rated for sessions lasting shorter or longer than 45-50 minutes.
This agreement is effective the date signed by both parties. This agreement may be terminated by either party using verbal, telephone or email communication and/or when therapy ends.
